Degenerative Disc Disease / Symptomatic Disc Degeneration / Spondylosis

Symptomatic disc degeneration, also known as spondylosis or degenerative disc disease is thought to be the leading cause of chronic low back pain. It can be caused due to dehydration of the disc. The disc shrinks and its loss in height increases stress on the facet joints of the spine causing them to degenerate and in the process increase in size. This places pressure on the nerve roots resulting in a sciatic type chronic pain.
Aging and injury are common causes of lumbar spondylosis and disc degeneration and by the age of fifty, 85 percent of the population will show evidence of this. The most common symptom is midline low back pain with or without radiation to the hips. Aching pain in the buttocks and or the backs of the thighs may be seen with walking.

Conservative treatment is successful 75% of the time. During the acute phase, anti-inflammatory agents, analgesics, and muscle relaxants will provide relief from pain and may be prescribed for a short period of time. The affected area may be immobilized and/or braced. Soft cervical collars may be used to restrict movement and control the pain. Lumbosacral orthotics may decrease the lumbar load by stabilizing the lumbar spine. In physical therapy, heat, electrical stimulation, and other modalities may be incorporated into the treatment plan to control muscle spasm and pain. Physical Therapy rehabilitation teaches the patient how to exercise and strengthen their paravertebral and abdominal muscles to lend support to the spine. Exercise in general helps to build strength, flexibility, and increase range of motion. Lifestyle modification may be necessary. This may include an occupational change (e.g. from manual labour), losing weight, and quitting smoking.
